A formal investigation by the Association of Professional Engineers & Geoscientists of Alberta (Apega) found the company was at fault when it did not ensure building plans for its tank roof-support structure were certified by an engineer or the contractor was competent. It was fined a maximum of C$10,000.

In April 2007 the tank roof-support structure of a high oil tank was being constructed at Horizon collapsed when the cables snapped due to high winds.
