The UK’s Health & Safety Executive (HSE) has served three improvement notices against the marine contractor following a three-day inspection in late October.

Teekay operates the vessel on behalf of UK supermajor BP and is responsible for safety. The HSE said Teekay Petrojarl failed to demonstrate that integrity testing of the temporary refuge — an area required by law on all offshore platforms to protect workers against fire and fumes in an emergency — had been carried out, despite commitments that this would be done after an earlier inspection in December 2015 found faults.
