A hydrocarbon leak occurred on 11 June from pipework caused by corrosion in a trunnion pipe support, according to the Health & Safety Executive (HSE).
“You have failed to ensure that trunnion pipe supports exposed to conditions causing deterioration which are liable to result in dangerous situations are inspected at suitable intervals, using appropriate techniques,” HSE said in the Prohibition Notice.
