NTSB: Shell failures led to Kulluk grounding

The 2012 grounding of mobile offshore drilling unit Kulluk in frigid Alaskan waters was the result of poor planning on the part of operator Shell, which bears ultimate responsibility for the accident, US transportation regulators have concluded.
