'Poor welding safety' led to Black Elk explosion

A lack of safety precautions for offshore welding, as well as poor communication among contractors, were among the driving factors that resulted in a deadly blast at a Black Elk Energy platform in the Gulf of Mexico last year, a US regulatory investigation concluded.
