An inspection by the Health & Safety Executive (HSE) found failures in the way staff were monitoring and reviewing inhibits and overrides on instrumented protective systems, according to an improvement notice, served on 28 July.
One requirement for the inhibits and overrides that were in place to be reviewed at least once every 14 days was not being met, the notice read.
