In the context of offshore safety, nothing in recent times springs to mind more quickly than Macondo. Five years after that seminal event, DNV GL has published a thorough summary of the findings and recommendations of 21 major inquiries carried out in that time (see table overleaf).
If a document that encompasses so much can be boiled down to a single idea, it might read — much has been done to reduce the risk of another major incident, but more change is needed.
