A “significant” release occurred on 4 May, according to an improvement notice just made public by the Health & Safety Executive (HSE).
HSE said ConocoPhillips failed to ensure that a low-pressure vent knockout drum was “constructed or adapted to be suitable for the purpose for which it was used or provided”.
Knockout drums are pieces of equipment used in flare and vent systems that remove liquid droplets from gas.
