The Petroleum Safety Authority (PSA) identified 10 non-conformities on the vessel, including the lack of a risk assessment and action plan, poor training of personnel, control of steel ropes and control of cranes and davits, after carrying an audit of lifting systems in December.
“It was disclosed during interviews that personnel carrying out important functions had little familiarity with applicable standards and activities for which standards are prescribed,” the PSA stated in its audit report.
