The global oil and gas industry has come a long way since the 1988 Piper Alpha disaster in strengthening its safety systems but major accidents and deaths continue to occur and sector leaders have been warned to guard against complacency and believing that all is “well and under control”.
Where the North Sea is concerned, deaths have been avoided, in some instances, almost certainly only because recent large gas releases did not find a source of ignition.
