A letter by the deputy chief 
counsel of the commission to the commission members said 
Chevron’s laboratory personnel were unable to generate stable foam cement using the materials Halliburton provided and suggested the foam cement used on the Macondo well was unstable.

“We have known for some time that the cement used to secure the production casing and isolate the hydrocarbon zone at the bottom of the Macondo well must have failed in some manner,” the commission letter said.
