The UK offshore watchdog has issued an industry-wide safety alert after the failure of a clamp led to a gas leak at the end of last year.
An incident occurred on an unnamed offshore production platform in December 2014 where a one-inch diameter Vector “Techlok” pipework clamp connector catastrophically failed, causing a gas release, according to the Health & Safety Executive (HSE).
