The UK’s offshore safety regulator, the Health & Safety Executive (HSE) would not reveal on which facility the incident took place or when.
However, it said it issued the alert to warn about incorrectly manufactured crane penants, the name given in the offshore industry to a method of attaching loads to cranes.
Pennants are considered safer offshore than conventional crane blocks because loads are less likely to swing around when installations or supply vessels are in motion.
