An Improvement Notice was issued on 21 June, according to the UK Health & Safety Executive (HSE).
The notice states that the arrangements in place for Shearwater’s control room operator, described as a “level 1 safety critical role”, are not as described in Shearwater’s safety case.
HSE said this meant Shell had failed to ensure that procedures and arrangements for competence assurance were being followed.
