The deceased had reportedly been working for 34 hours when the accident happened, a fact that is not mentioned in the investigation report. Also, eyewitnesses said that the 200kg pipe that hit him did not fall through the mousehole, as stated in the report, but through a larger uncovered opening in the drillfloor.
The Norwegian Petroleum Directorate (NPD) has previously blamed oil company Statoil, which was in charge of the drilling operation, and drilling contractor Dolphin -- a subsidiary of Fred Olsen Energy -- for a number of faults in the run-up to the fatal accident.
