The deep-water Macondo well blew out in a fiery rage on board the semi-submersible Deepwater Horizon on 20 April 2010, killing 11 workers and resulting in one of the nation’s worst oil spills.
The contractor’s investigation team traced the causes of the blowout to four over-reaching issues: risk management and communication, well design and construction, risk assessment and process safety and operations.
